Denton Family Practice Clinic (a DBA of Health Express Clinics’ Network PLLC)

1. Acceptance of Terms
These Terms of Service (“Terms”) govern your use of the website located at
https://dentonfpc.com (the “Website”), operated by
Health Express Clinics’ Network PLLC, doing business as Denton Family Practice Clinic
(“Denton Family Practice Clinic,” “we,” “us,” “our”). By using the Website, you agree to
these Terms. If you do not agree, please do not use the Website.
2. Eligibility
The Website is intended for use by individuals 18 years of age or older. Minors may not
use the Website or submit forms on it directly. If you would like to request an appointment,
refill, or other service on behalf of a minor patient, a parent or legal guardian must submit
the request or contact our office by phone.
3. Description of the Website
The Website provides general information about Denton Family Practice Clinic, our
providers, and our services, and allows visitors to submit non-urgent requests, including:

- Medication Refill Request Form
- Contact Us Form
Submitting a form through the Website is a request only — it does not guarantee an
appointment, refill, or specific response time. Our staff will contact you to confirm.
4. Not for Emergencies; No Medical Advice
The Website, our texting program, and our AI voice receptionist are not monitored 24/7
by clinical staff and are not intended for urgent or emergency use. If you are experiencing
a medical emergency, call 911 or go to the nearest emergency room.
Content on the Website is provided for general informational purposes only, does not
constitute medical advice, and does not create a physician-patient relationship. A
physician-patient relationship is established only once you have been formally registered
and seen as a patient of the practice.
5. Medication Refills
Medication refill requests submitted through the Website are subject to our standard
refill policies, available at

including required processing time, office-hours-only handling, and in-office visit
requirements for certain medications.
6. Text Messaging Program
If you opt in to receive text messages from us:
- You consent to receive messages related to your care, such as appointment reminders and refill notifications, at the mobile number you provide.
- Message frequency varies based on your appointments and requests. Message and data rates may apply.
- Reply STOP to opt out at any time, or HELP for help.
- Consent to receive text messages is not a condition of receiving care or any service from Denton Family Practice Clinic.

7. AI Voice Receptionist Calls
If you interact with our AI voice receptionist by phone, or opt in to receive automated
calls from us:
- Calls may be answered, or in some cases initiated, by an artificial-intelligence voice agent rather than a live staff member.
- These calls may be recorded and transcribed for scheduling, recordkeeping, and service-improvement purposes.
- You may ask to speak with clinic staff at any point during a call.
- Consent to receive automated calls is not a condition of receiving care or any service from Denton Family Practice Clinic.
- Use of the AI voice receptionist does not create or change the physician-patient relationship, which is established only through your care with our providers.
- If you are experiencing a medical emergency, hang up and call 911.
8. Patient Portal
Our Patient Portal is operated by a third-party platform and is governed by that
platform’s own terms of use, separate from these Terms. We are not responsible for the
availability or functionality of the Patient Portal.
9. Acceptable Use
When using the Website, you agree not to:
- Submit false, misleading, or fraudulent information through any form
- Attempt to interfere with the security or normal operation of the Website
- Use the Website to submit spam, unsolicited messages, or unlawful content
- Attempt to access areas of the Website not intended for public use
10. Intellectual Property
All content on the Website — including text, graphics, logos, and images — is owned by
or licensed to Denton Family Practice Clinic and may not be copied, reproduced, or
distributed without our prior written permission, except as necessary to use the Website
for its intended purpose.

insurance provider websites, Google Maps, and our social media pages. We do not control
and are not responsible for the content, policies, or practices of those third parties.
12. Disclaimer of Warranties
The Website is provided “as is” and “as available,” without warranties of any kind,
whether express or implied, including warranties of merchantability, fitness for a
particular purpose, or non-infringement. We do not warrant that the Website will be
uninterrupted, error-free, or secure.
13. Limitation of Liability
To the fullest extent permitted by Texas law, Denton Family Practice Clinic and Health
Express Clinics’ Network PLLC will not be liable for any indirect, incidental, special, or
consequential damages arising from or related to your use of the Website, our texting
program, or our AI voice receptionist, even if advised of the possibility of such damages.
Nothing in these Terms limits any liability that cannot be limited under applicable law,
including liability related to the professional care you receive from our providers, which is
governed separately by the terms of your care and applicable Texas medical liability law.
14. Indemnification
You agree to indemnify and hold harmless Denton Family Practice Clinic, Health Express
Clinics’ Network PLLC, and their owners, employees, and agents from any claims, damages,
or expenses (including reasonable attorneys’ fees) arising from your misuse of the Website
or violation of these Terms.
15. Dispute Resolution and Governing Law
These Terms are governed by the laws of the State of Texas, without regard to its
conflict-of-laws principles.
Any dispute arising out of or relating to the Website will be resolved exclusively in the
state or federal courts located in Denton County, Texas, and you consent to the personal
jurisdiction of those courts.
These Terms do not include an arbitration requirement — disputes are resolved through
the court system described above.
16. Changes to These Terms
We may update these Terms from time to time. The “Effective Date” above reflects the
most recent revision. Continued use of the Website after changes are posted constitutes
acceptance of the revised Terms.
17. Severability
If any provision of these Terms is found unenforceable, the remaining provisions will
remain in full force and effect.
18. Entire Agreement
These Terms, together with our Privacy Policy, constitute the entire agreement between
you and Denton Family Practice Clinic regarding use of the Website.
